Requirements to establish a new account

  • Completed Application for Utility Service
  • Acknowledgment and Release of Liability form (waiver)
  • Copy of Driver’s License of primary account holder
  • Closing Settlement (HUD) with signatures of buyer and seller or Lease Agreement
  • For residential account, a deposit of $100.00. Pay with a check or money order payable to SLWSD
  • If paying deposit in the office with a credit card or debit card, please now this type of payment will incur a processing fee of 3.10%
  • For commercial account information, please contact the district at (772) 340-0220 for details on the account and amount of deposit needed
  • One (1) business day notice is required to establish an account
